Anyone—administrators, board members, water operators, and other community members—may nominate a PWS that
they think deserves the Small Water Systems Security Program Award, Substantial
Source Water Protection Program Implementation Award, Innovative or Proactive System Award,
Outstanding Public Drinking Water System Award, or 
Outstanding Cross-Connection Control Program.
The easiest way to nominate a PWS for one or more of these awards is to complete and submit a nomination form. Use the
version that is most convenient for you:
- MS Word form (Help with
Downloading Files.) Save this version to your computer, complete the blanks, save it,
and e-mail a copy to pdws@tceq.state.tx.us with “PDW Recognition Nomination”
in the subject line of your e-mail. You may also print out the completed form and submit it by regular mail or fax.
- PDF file (Help with PDF.) Save this version to your computer, complete the blanks, print it out, and submit
it either of these ways:
- Regular mail. Send a completed nomination form (Word
or PDF) to:

- Fax your completed nomination form (Word
or PDF) to 512-239-6050 with “Attn: PDW
Recognition Program” on the cover page.
If you would rather not complete a nomination form, simply send an e-mail to pdws@tceq.state.tx.us
with “PDW Recognition Nomination” in the subject line. In your e-mail, be sure to include this information:
- Name of the PWS
- Award you are nominating this PWS for
- A brief description of the accomplishments that qualify this PWS for this award
You may also send us the above information in a letter to the address shown above. Nominations for recognition
for 2007 must be received by April 1, 2008.
