Certificate of Delivery of Public Notice to Customers for All Public Notice Types

After issuing a public notice associated with any of these types of violations:

For Community Public Water Systems:
- a chemical violation (acute or nonacute)
- a microbial violation ( acute or nonacute)
- a treatment technique violation
For Noncommunity PWSs:
- a chemical violation (acute or nonacute)
- a microbial violation ( acute or nonacute)
- a treatment technique violation

complete this certificate and submit it to the TCEQ with a copy of the notice issued.
